One Denture - $24.95/mo per denture

Coverage Details
Full denture coverage includes:
- All repairs and adjustments (per Adjustment Schedule)
- 1 hard reline and 2 soft relines per 3 year period, to be used at any time*
- Ultrasonic cleaning and lab polish of the denture up to 4 times per year
- 2 exams per year
- 2 business days (additional business days may apply if shipping is required) denture replacement of lost or destroyed dentures $150
- New high-quality denture every 5 years
- Additional denture services 25% off normal fees (such as additional relines). Does not include implant services.
- 10% off dental implant services after 1 year (in addition to any promotions)
Partial denture coverage includes:
- Cast partial dentures only
- All adjustments and repairs, including the addition of wrought wire clasps (cast clasps are not covered)
- 1 hard reline and 2 soft relines per 3-year period, to be used at any time*
- Ultrasonic cleaning and lab polish of the partial denture up to 4 times per year
- 2 exams per year, including x-rays
- 10% off tooth cleanings and periodontal procedures
- 10% off of dental services, such as fillings and crowns, after initial treatment plan
- Replacement of lost or destroyed cast partial dentures for $300
- Replacement time for cast partial dentures varies depending on the steps required to fabricate a new restoration
- New cast partial denture every 7 years.
- Additional denture services 25% off normal fees (such as the addition of a tooth). It does not include implant services.
- 10% off dental implant services after 1 year (in addition to any promotions)
* After initial paid hard reline (if indicated in the treatment plan for an immediate denture) one hard reline is included to be used at any time within 3 years after the initial reline. An additional hard reline is added each 3 years. 2 soft relines are included to be used at any time in the 3 years following subscription set up, and 2 additional soft relines are added each 3 years.
Connector (Abutments) coverage includes:
- Covers Equator connectors (Rhein 83) and Locator connectors (Zest)
- Replacement of retentive nylons up to twice a year
- Tightening and maintenance of connectors
- Polishing of connectors at exams
- Repair or replacement of damaged or lost metal housings
- Replacement of worn connectors
- Wear of greater than 25% of the connector
Dental implant coverage includes:
- Covers dental implants used to retain removable full and partial dentures
- Replacement of failing dental implants
- 2 exams per year including radiographs of the implants
- 2 x scaling/polishing of implants per year and additional 10% off full tooth cleaning or periodontal services
- Required soft tissue grafts that were not diagnosed within the first year after restoration are 25% discounted rate
- Lifetime implant warranty requires a minimum of 1 clinical exam, necessary radiographs, and 1 cleaning/polishing of implants per year
- Dental implants that require replacement will be replaced with the brand of dental implant that is being placed by the Denture Club of America at the time of replacement
- A failing implant is defined as:
- Mobile implant
- Encapsulation by soft tissue
- More than 1/3 of the implant is supra-crestal (above bone)
- Chronic peri-implant infection (more than 3 episodes of infection/drainage in a year)
